'They originally planned to kill off Ianto??'

I can't remember where I read it but, apparently, Ianto was suppose to be a bit part, and they planned to kill him off during Cyberwomen (that's why he doesn't have a lot of screen-time). Gareth did such a great job with the role that they decided not to :D
