My gut reading of it was definitely that it was about her age. In fact, initially I thought he was telling Donna she was too fugly to have feminlne wiles, but then my brain refused to accept that that was something the Doctor would say, so I settled on the age thing. The implication was definitely that he wasn't talking about a character trait, but about a superficial one.
